The Toowoomba Hospice is a six-bed private hospital and healthcare facility dedicated to providing optimal, client-centred palliative care, free of charge.
Clients aged 18 years and over may be admitted to the Hospice for end-of-life care if they:
Have a life-limiting illness or health condition;
Are no longer receiving active medical or curative treatment; and
Have been assessed by their medical practitioner, specialist, or nursing practitioner as being within the last three months of life.
How to Access Our Services
Admission to the Toowoomba Hospice can be arranged through:
A direct request from the client or a family member;
A referral from the client's General Practitioner (GP) or specialist; or
A referral from the client's treating hospital.
All referrals are assessed by the Admitting Clinical Nurse or Director of Nursing. Referrals are triaged according to clinical need, with priority given to community-based clients and those requiring end-of-life care who have multiple or complex health needs.
We encourage prospective clients and their families to visit the Hospice prior to admission whenever possible. This provides an opportunity to view our facilities, meet our caring team, and discuss any questions or concerns.
